Eligibility Criteria

  • Native Hawaiian Ancestry
  • Acceptance and full-time enrollment into an accredited program from one of the following disiplines:
    • Clinical Psychology (PhD or PsyD) 
    • Dentistry (DDS or DMD)
      • Approved Specialties: General Practice Dentistry, Advanced Education in General Dentistry, Pediatric Dentistry, Public Health Dentistry
    • Dental Hygiene (BSDH or ASDH) 
    • Dietetics/Nutrition (MSDD) 
    • Nursing, Nurse-Midwifery and Nurse Practitioners (DNP, MSN, NM or NP, BSN, ADN) 
      • Approved Specialties: Family, Psychiatric Mental Health
    • Physician (MD or DO)
      • Approved Specialties: Family Medicine, General Internal Medicine, General Pediatrics, Opthalmology, Obstetrics-Gynecology, General Psychiatry, Internal Medicine/Family Medicine, Internal Medicine/Pediatrics, Family Medicine/Psychiatry, Internal Medicine/Psychiatry, Rotating internship with a request to complete
        a residency in one of the above specialties (DO
        only)
    • Physician Assistant (MSPA) 
    • Social Work (MSW)

How to Apply

Required Documents

  • Focused Resume
    • Format Requirements 
      • Max 2 pages 
      • 12-point font or larger 
  • College/University Proof of Application or Letter of Acceptance
    • PDF Format
    • (If Applicable) Upload each proof of application if you have applied to multiple College/Universities.
  • Proof of Native Hawaiian Ancestry 
    • Applicant's Certificate of Birth that meets the following criteria: 
      • Verifies Native Hawaiian Ancestry 
      • Scanned Front and Back Colored Original with embossed seal (Black and White will NOT be accepted) 
        • Note: If Hawaiian ancestry is NOT listed and birth certificate does not state “Hawaiian,” applicant will also include certified birth certificates of each family member until “Hawaiian” is on the birth certificate (see item #2). 
        • Note: If applicant's name does NOT match the original certificate of birth, applicant will also include copies of documents demonstrating such name change (marriage certificate / legal name change).
    • (If Applicable) Applicant’s Native Hawaiian Parent/Grand Parents/Great Grand Parent's Certificate of Birth. If Hawaiian ancestry is NOT listed and birth certificate does not state “Hawaiian,” applicant will also include birth certificates of each family member until it verifies Native Hawaiian Ancestry or states “Hawaiian” on the birth certificate and meets the following criteria: 
      • Scanned Original with embossed seal 
      • Front and Back 
      • For Example: Applicant’s Original Certified Birth Certificate, Mother’s Original Certified Birth Certificate, Maternal Grandmother's Original Certified Birth Certificate, etc.
    • IMPORTANT REMINDER: Letters from Ho'oulu Data Center or Office of Hawaiian Affairs (OHA) registry are NOT acceptable forms of verification.
  • Most Recent Transcript 
    • PDF Format
    • Unofficial transcripts will be accepted
  • Degree Pathway
    • Provide ONE of the following (PDF format only):
      • Official degree plan from your academic program, OR
      • Academic plan or program of study signed by your advisor
    • What your document must show:
      • Course codes and course titles for each term
      • Credit hours for each course
      • Total credits per term
      • Total credits required to complete your degree
      • Which terms will you be enrolled in (e.g., Fall 2025, Spring 2026, etc.)
    • If your program spans multiple years: Your degree pathway must cover your entire program from first enrollment through graduation, not just one academic year.
  • Recommender Contact Information (Name and Email) for Academic Advisor and Employer
    • Full recommendation letters will NOT be accepted. A recommendation form will be available through the online application instead.
    • Recommendation from a Academic Faculty or Advisor is mandatory. This recommendation form should speak to your academic progress and commitment to your chosen profession.
    • If you are employed, a recommendation form must come from your employer addressing your work ethics and behavior. If you are not employed, then the letter of recommendation may come from a community organization which you volunteered for.
